a été hospitalisé dans notre service du () au () devant la crainte dun nouvel accident ischémique.
Les antécédents et facteurs de risque vasculaire sont déjà charges comprenant : HTA, diabète de type II, cardiopathie ischémique stentée, tabagisme récemment arrêté, surpoids, artérite sévère des membres inférieurs ; enfin un tout récent accident ischémique capsulo-thalamique gauche fin aout.
Cette fois il rapporte de façon pas brutale mais rapidement progressive sur 30mn dun déficit sensitivo-moteur gauche chez un gaucher, lensemble ayant duré quelques heures.
Initialement le soir même ce 9 septembre est encore constate un discret déficit brachio-facial gauche, une petite note également proprioceptive et il rapporte toujours quelques paresthésies de lhémicorps gauche.
La TDM complétée dune IRM encéphalique nobjective pas de nouvelle lésion ischémique en particulier à droite mais les mêmes lésions que celles constatées fin aout au décours du premier accident ischémique à savoir des plages dinfarctus bi focaux : de la cérébrale antérieure gauche (petit infarctus paramedian) et capsulo-thalamique gauche ; les séquences agiographiques étant sans particularité. Dans le même ordre didées, lécho doppler cervico-encéphalique objective une athéromateuses diffuse avec plus particulièrement une sténose de la carotide interne droite estimée entre 50 et 55% (cote asymptomatique).
Dans l'hypothèse d'un éventuel déficit post critique, un EGG a été réalisé qui savère sans particularité.
LEGG déroule un rythme sinusal. La biologie standard est sans particularité, lhémoglobine glycosylée est un peu élevée à 7.2%.
On constatera ensuite régulièrement durant son séjour des épisodes dhypoglycémie autour de 0.6,0.7g/l, pauci symptomatique et je pense que cest lhypothèse qui doit prévaloir ; la description clinique est en effet peu compatible avec un accident ischémique devant le caractère progressif de la sémiologie et dautre part, alors même que cette sémiologie a été prolongée, il ny a aucun stigmate ischémique à droite en diffusion.

His Medical History and identified vascular risk factors revealed the following: high blood pressure (HBP), type 2 diabetes, ischemic heart disease treated with stents, smoking quitted recently, overweight, severe arteritis of the lower limbs and a very recent left-sided capsule and thalamic ischemic stroke attack which occurred in August ending.
Presently, with a rather steady than sharp progress, it takes 30min for the left-handed individuals sensitive and motor deficit to occur, the whole process lasting for few hours.
Initially, on that very evening of September 9, a subtle left-side brachio-facial deficit and a small proprioceptive sense were noticed and a continuous paraesthesia of the left hemibody.
The brain MRI and CT scan conducted do not reveal new ischemic lesions on the right side but rather lesions similar to those identified in August, after the first ischemic stroke incident. They are bifocal heart attack symptoms, an affected area of the left brain anterior (a small paramedian infarct) and left capsule and thalamic haemorrhage symptoms with insignificant angiographic sequences. In the same vein, the Transcranial Doppler Ultrasound test conducted searched for atheromatosis that has spread, specifically with a left inner carotid artery stenosis estimated at between 50 and 55 percent (asymptomatic side).
An EEG, with insignificant results was done in the event of a possible post critical deficit.
ECG showed a normal sinus rhythm. Standard biology was insignificant, while glycosylated haemoglobin slightly rose to 7.2 percent.
During his stay, pauci-symptomatic hypoglycaemic episodes around 0.6 and 0.7 g/l were recorded on a regular basis and I believe the hypothesis must prevail. Clinical description is less compatible with an ischemic stroke given the gradual nature of semiology. Even if further semiology studies are done, there will be no right-side ischemic stroke-related stigma.

Les termes commençant par une majuscule utilisée dans le Contrat auront la même signification que celle qui leur est donnée en langue anglaise dans la Convention de Crédit, sauf s'ils sont définis différemment dans le Contrat.

un avenant doit s'interpréter comme comprenant tout accord modificatif ou complémentaire, toute novation, réitération ou renouvellement et modifié doit être interprété en conséquence ;
un Article, un Paragraphe ou une Annexe est une référence à un article, un paragraphe ou une annexe du Contrat ;
une disposition législative est une référence à cette disposition telle quappliquée, modifiée ou codifiée et inclut tout décret et autre norme d'application ;
une personne inclut ses successeurs, cessionnaires, ayants causes et ayants droits autorisés ;

Interpretation
Terms used beginning in capital letters shall have the same meaning as those given to them in English in the Facility Agreement, except where they have different definitions in the Agreement.
In the Agreement, unless otherwise stated or the context requires a different meaning, referring to:
an amendment should be understood as comprising any amending or supplementary agreement, any modification, reiteration or renewal and should consequently be interpreted as such;
an Article, a Paragraph or an Annex shall be a reference to an article, a paragraph or an annex of the Agreement;
a legislative provision shall be a reference to this provision as applied, modified or codified and shall include every decree and other application norms;
an individual shall include his authorized successors, transferee, assignees and assigns;
the references made in every contract or document shall be interpreted as applying to this agreement or document such that this could be amended, modified, completed or novated at any moment;
an ongoing Enforcement Event shall refer to Enforcement Event which has not been addressed or has been abandoned; and
Finance Documents (or any other document) shall comprise (without prejudice to any prohibition to modify) any modification, regardless of its significance, made to each of these Finance Documents (or to the document considered) and notably, any amendment stating an increase in the amount of a facility or an additional facility
Every Annex shall be an integral part of the Agreement.
The words appearing shall include the singular and vice versa.

Born and bred in Yaounde to English-speaking parents, I became conversant with the English and French
languages early in life thanks to the bilingual nature of my country.
Undertaking studies under the British system of education
right from the primary level, in the year 2000, I moved to Bamenda in the English-speaking
part of Cameroon, where I pursued secondary and high school studies.
