If you are sick and there is a medicine that would make you/get you "di nuovo sano come un pesce", would it be "fit as a fiddle", "sound as a bell", "back in top form"?
Fit as a fiddle has for me other connotations, more to do with fitness. Any suggestions?

"fit as a fiddle" sounds a bit old-fashioned to me

Here's another suggestion, maybe a little less old-fashioned than fit as a fiddle.
Or you could say that someone will soon be feeling in tip-top shape again.
