medical certificate
That is what I would ask any English speaking doctor for anywhere in the world. If the NHS is currently calling it something stupid, that is only to be expected of the poor bureaucracy encumbered organisation who have nothing better to do except dream up new names for bits of paper.

A medical certificate (sometimes referred to as a doctor's certificate[1][2]) is a statement from a physician or other health care provider that attests to the result of a medical examination of a patient.[3] It can serve as a "sick note" (documentation that an employee is unfit for work) or evidence of a health condition.[4]

sick note
I think this is what the vast majority of the English-speaking world would call it.

Fit Note
I have supplied a link to the NHS website for the UK. This used to be called a sick note but the terminology has changed and is now a fit note. The Doctor will sign if an employee's illness has lasted for more than 7 days (if under 7 days the employee should use a self certification form that most companies provide or that can be purchased from HMC ( Employee's Statement of Sickness).

From the website depending on which choice you feel would be most appropriate for your translation style a fit notes may also be called medical statements or a doctor's note.

Example sentence:
f you're off work sick for more than seven days, your employer will usually ask you to provide proof that you've been ill. They will normally ask for a fit note from your GP.

: NHS or no NHS, I think it's confusing to call it a "fit note".//But this is about Italy, not the UK. What the NHS calls it is not relevant.

If that is the terminology used at the present time then I do not see why it would be confusing. It is merely a question of altering one's mindset. And the NHS is the largest provider of medical care in the UK so not to be totally ignored, I think.

: Though it has the same meaning (though not everywhere) this term does not carry the same formality as the source language term used.

Agreed that this would depend on the country and usage but sick note, fit note, medical certificate choices would depend on context. I have offered alternatives to fit note in my comments. Thank you Phil
