A report from England in 1992 described pharyngeal infections with tox– ***C. diphtheriae*** developing predominantly among homosexual men; they often caused symptomatic pharyngitis and sometimes resulted in a ***tonsillar exudate***.
В 1992 г. вспышка дифтерии зева (ангина, иногда с ¶налетами на небных ¶миндалинах) разразилась в Англии среди гомосексуалистов. Она была вызвана нетоксигенным штаммом Corynebacterium diphtheriae.

Unlike the ***exudate*** in streptococcal pharyngitis, the ***diphtheritic pseudomembrane*** often extends beyond the margin of the tonsils onto the tonsillar pillars, palate, or uvula. Dislodgement of the membrane is likely to cause bleeding. Estimates of the proportion of patients with pharyngeal diphtheria who develop typical pseudomembranes vary widely, from as few as one-third to almost all.
В отличие от стрептококковой ангины, при дифтерии ¶налет часто распространяется за пределы небных ¶миндалин — на дужки, небо и язычок. При попытке снять пленку нередко возникает кровотечение. Эта форма дифтерии зева, называемая пленчатой, по разным оценкам, наблюдается в 33—100% случаев.

Tonsillar diphtheria. Membrane is confined to the tonsils, where absorption of toxin is moderate. It starts as a small ***patch*** on one tonsil and usually spreads to both. The membrane is ivory white or greyish yellow in colour.
Локализованная дифтерия зева. Фибринозной пленкой покрыты только миндалины. Всасывание токсина незначительно. Вначале ¶налеты появляются ¶островками на одной миндалине, в дальнейшем обычно распространяясь на обе. Фибринозная пленка цвета слоновой кости либо серовато-желтая.

Anginose variety – follicular ***exudate***. As the illness progresses ***patches of white exudate*** appear on the tonsils, which may be very swollen. The uvula is red and oedematous.
Ангина: ¶островки ¶налета на миндалинах. Через некоторое время на миндалинах появляется белый налет. Миндалины и язычок гиперемированы и отечны.

A characteristic pseudomembrane on the mucosa of the oropharynx, palate, nasopharynx, nose, or larynx suggests diphtheria but is not uniformly present. ***Diphtheritic pseudomembrane*** must be distinguished from other pharyngeal exudates, including those of group A beta-hemolytic streptococcal infections, infectious mononucleosis, viral pharyngitides, fusospirochetal infection, and candidiasis.
Признак ¶дифтерии — характерные ¶налеты на слизистых ротоглотки, неба, носоглотки, носа или гортани, но они бывают не всегда. ¶Дифтерийные пленки приходится дифференцировать с ¶налетами на небных миндалинах при стрептококковой ангине, инфекционном мононуклеозе, вирусных фарингитах, ангине Венсана, кандидозе.
