Это из сноски - положение по штату Мериленд (я думаю, это распространяется повсеместно). Paramedics тут не годится.
Вторая сноска - про моих провинциальных парамедиков. Кстати, у нас тем, кто работает на скорой, платят только если поступает вызов (т.к. с вызвавшего "отщелкивают" 100 долларов за удовольствие).

I think that for the piece in question, this translation works best as the topic is mother-to-child transmission of HIV.
I am however still undecided as to whether it could also be translated as 'paramedics'. I had always thought that they were emergency medical personnel who work from ambulances but it seems that indeed that is not always the case.
