Is this the 1st, as opposed to the 2nd, branch of the ramus diagonalis?

I think you're inferring too much Yes, usually D1 is the strongest. But this might not be the case here. I'd stick with how the doctor has described it.

There's this link for example - maybe it's the only example! - that led me to believe that perhaps "silný RD" means "RD1":

descriptions I would translate it as the doctor has written it - not designating which diagonal branch it is. --> "in the middle strong diagonal branch".

They're talking about arteries here. Also: "...emergentní SKG s průkazem asymetrické léze proximální RIA charakteru intramurálního hematomu s přechodem do silného RD".

(central) strong diagonal branch

Unless the report provides more information somewhere else, we do not know the number of diagonal branches the patient has. Based on the below reference, there may be often 2 to 9 of them, the 1st one usually being the most prominent. I would therefore translate literally.

"Diagonal branches of the left anterior descending coronary artery supply blood flow to the anterior and anterolateral walls of the left ventricle. There are usually denoted as D1, D2, D3, etc.

"There are termed "diagonal" due to them branching from their parent vessel at acute angles. They extend over the left ventricle in a diagonal fashion toward the acute margin and the cardiac apex. They often run parallel to one another and are variable in number (often 2 to 9)... The first diagonal (D1) branch tends to be the most prominent. When the first diagonal is large, the other diagonal vessels tend to be small and run a shorter course."
