First, yes I am aware that there already is an entry for Schwerpunktkrankenaus, but I wonder if the answer, specialized hospital, is the right one.
The text I am translating differentiates between what seems to be a more general reference to a 'Fachkrankenhaus' and the term 'Schwerpunktklinik,'
which I think is a new, specific term. It says the first Schwerpunktklinik was certified in Berlin in 2018.
Duden defines a Schwerpunktkrankenhaus as 'Krankenhaus, das sich schwerpunktmäßig auf bestimmte Arbeitsbereiche konzentriert.'
Context: Im Juni 2018 wurde die Caritas-Klinik XX als damals erste Schwerpunktklinik Berlins zertifiziert. Berlinweit sind es mittlerweile fünf, bundesweit 58 HFU (heart failure unit)-Schwerpunktkliniken.
I am using 'hospital focusing specifically on congestive heart failure' but would appreciate any more elegant proposals!

I would suggest to combine the previous suggestions.
IMO, neither "tertiary" nor "specialist/specialty" alone are sufficient because a tertiary facility provides highly technical care in several specialties, plus general care, and a specialist facility provides service at any level in a given specialty. Moreover, it can't be said that the first "tertiary center" was certified in Berlin in 2018 – they have existed long before.
"Schwerpunktklinik" seems to be a term created for the German health care system, and depending on your context, you might consider to leave it untranslated and optionally add an explanation in brackets.
