Index problems in MS Word 2010

I am making an index with MS Word 2010, and have two problems. One is that one (and only one) item with sub-entries repeats each sub-entry name in the index, rather than grouping them together. All the other entries with sub-entries are fine. The second problem is that the very last word in the index starts with A! (It follows a word that starts with Z.) Everything else about the index is fine.
