Granulomatosi di Wegener, episodio in Maggio 2011 di angina instabile con trombo endoluminale dell'IVA, episodio di IMA nel 2007, nel 2007 episodio di occlusione arteria centrale della retina, pregressa pachimeningite, impegno di naso e seni paranasa!i e pregressa otite, ipogammaglobulinemia corretta con infusioni mensili di lg e.v.
Terapia:
Medrol 16 mg cp da Aprile 2018
Leflunomide 20 mg 1 cp ogni 3 giorni
Peptazol 40 1 cp (talora 1 cp x 2)
Domperidone 10 mg 1 cp prima dei 2 pasti
Didrogyl 10 gtt
Nerixia 25 mg 1 fl im
Contramal 100 mg cp (per lombalgia, ma lo assume al bisogno)
CardioASA 1 cp
Piavix 75 1 cp
Congescor 2.5 mg
Tachipirina S.Q. Raramente
Riferisce da tempo episodi acuti di secchezza diffusa oculare nasale orale e cutanea., che si associano a aumento artralgie a mani polsi caviglie + astenia. Dorme male e ha spesso una sensazione di "tremore" non obietti vabile. Ha smania notturna arti inferiori. Ha avuto nuovamente piccolo ascessi cutanei trattati localmente con clindamicina senza miglioramento sostanziale. Non può più fare levofloxacina con cui la volta scorsa ha avuto tendinopatia.
La RMN encefalo è risultata invariata, a liveilo rachide cervicale presenti discopatie con bulging.
Si consiglia:
esami ematici compresi ANA ENA DNA che farà il 3/7 in occasione prossima terapia
Rivotril 5 gocce prima di andare a letto in poca acqua (se necessario salire di 1 goccia ogni 2-3 giorni max 10 gocce )
Bassado 100 mg 1 cp al giorno dopo un pasto per 10 giorni
da programmare rituximab (Mabthera) a Settembre Ottobre
Prossime infusioni Immunoglobuiine il 3/7 e 31/7 ore 11.30

Wegener's granulomatosis, episode in May 2011 of unstable angina with endoluminal thrombus type IVa MI, episode of MI in 2007, in 2007 episode of central retinal artery occlusion, previous pachymeningitis, engagement of nose and paranasa!i sinuses and previous otitis, hypogammaglobulinemia corrected with monthly infusions of lg I.V.
Therapy:
Medrol 16 mg tab from April 2018
Leflunomide 20 mg 1 tab every 3 days
Peptazol 40 1 tab (occasionally 1 tab x 2)
Domperidone 10 mg 1 tab before 2 meals
Didrogyl 10 gtt.
Nerixia (Neridronate) 25 mg 1 IM
Contramal 100 mg tab (for low back pain, but takes it as needed)
CardioASA 1 tab
Plavix 75 1 tab
Congescor 2.5 mg
Tachipirina PRN Rarely
Has been reporting acute episodes of diffuse ocular nasal oral and skin dryness for some time., which is associated with increased arthralgias in hands wrists ankles + asthenia. She sleeps poorly and often has a sensation of an unobjectionable "shaking" feeling. She has nocturnal lower limb restlessness. She again had small skin abscesses treated locally with clindamycin without substantial improvement. She can no longer do levofloxacin with which she had tendinopathy last time.
Encephalic MRI was unchanged, at cervical spine level a presence of discopathy with bulging.
We advise:
Blood tests including ANA ENA DNA that she will do on 3/7 at the next therapy
Rivotril 5 drops before bedtime in little water (if necessary go up 1 drop every 2-3 days max 10 drops)
Bassado 100 mg 1 tab daily after a meal for 10 days
To be scheduled rituximab (Mabthera) in Sept. Oct.
Next Immunoglobulin infusions on 3/7 and 31/7 11:30 a.m.
