This is the context. Obviously literally, an "axe blow" appearance, but that doesn't come up anywhere or seem to fit. Any help would be appreciated.

I understand that a clinical examination does not involve imaging. I cannot agree that 'aspect' necessarily refers to medical imaging. I think Liz has it right.

This is definitely a clinical examination report and "aspect" correponds to imaging.

so this note is about what's been observed at clinical examination, not a patient's report of subjective symptoms.

@Dr Manu, Thanks for your answer. However I believe gapping is a sign that can be detected by palpation (I now think this is what is meant by "coup de hache" (see my ref). The Full thickness Achilles tendon tear in the ref you give was shown on MRI. I was therefore asking whether the diagnosis you have given as your answer could be made by clinical examination? It would also be useful to know from the asker how this observation waés made (clinical exam, imaging etc.).

I assume we're talking about the same thing here:
Inflammation of the retrocalcaneal bursa is caused by repetitive motion of the Achilles tendon over the posterosuperior aspect of the calcaneus or by pressure of the heel counter, or by a combination of both. Dorsiflexion of the ankle precipitates the pain. Radiographs may help identify the anatomic cause, a hatchet-shaped posterior calcaneus. A Fowler and Philip angle of greater than 75° or more is abnormal.

In case anybody, like me, is intrigued by the word 'dell':

Clinically, patients have increased edema and ecchymosis to the injured area. A gap or defect referred to as a “palpable dell” is present at the location of the rupture.

It doesn't mean there has been a hatchet strike. For whatever reason, it is an accepted term for an indentation at the back of the ankle. See my links.
disagree Daryo : "sensation de coup de hache douloureux derrière la cheville" is is a **subjective** symptom NOT a description of what's uncovered by physical examination - this ST is about **external observation** as per the clue hiding in plain sight: "on note ..."

it shows a lateral indentation just below the point of the shoulder. In the case of the heel this would be a median indentation caused by the tear
